I have had more than one client arrive at my chamber after an emergency room visit, convinced they were having a heart attack, only to be told their heart was fine and to see someone about stress. That is a genuinely common way anxiety first shows up, as a racing heart, tight chest, dizziness and a real fear that something is physically wrong. It usually takes a while before someone connects that experience to anxiety at all.
How common this actually is
The World Health Organization estimates that anxiety disorders affect over 300 million people globally, making them the most common mental health condition worldwide. There is no reason to think Bangladesh is meaningfully different, and Bangladesh’s own 2019 National Mental Health Survey found mental health conditions, anxiety among them, in close to 17 percent of adults. Most of those cases go untreated for years, partly because physical symptoms get chased through cardiology and gastroenterology before anyone considers anxiety as the cause.
What actually helps, based on the evidence
Cognitive Behavioral Therapy has strong, consistent evidence for anxiety disorders, among the strongest evidence bases of any psychotherapy for any condition. A 2023 meta analysis in Current Psychiatry Reports confirmed meaningful CBT effects across recent literature on anxiety related disorders, and a separate 2023 meta analysis focused on low intensity CBT for generalized anxiety disorder found medium sized reductions in both anxiety and worry compared to usual care. For panic disorder specifically, CBT that includes gradual exposure to feared physical sensations tends to outperform CBT without that component. Medication, typically prescribed by a psychiatrist, is also evidence based and is often combined with therapy for moderate to severe cases rather than used as an either or choice.

What does not reliably help
Avoidance feels like relief in the moment but tends to make anxiety worse over time, because it confirms to the brain that the situation was dangerous enough to avoid. Reassurance seeking, repeatedly asking others if things are going to be okay, works the same way. Both are worth mentioning honestly, since they are the instinctive responses almost everyone reaches for first, and they are part of what keeps anxiety going rather than resolving it.

What the latest research says
A 2024 network meta analysis by Papola and colleagues in JAMA Psychiatry pooled 65 randomized trials with 5,048 adults with generalized anxiety disorder. CBT and third wave CBT both reduced symptoms compared with usual care, but only CBT stayed clearly more effective 3 to 12 months after treatment ended. The authors concluded that CBT may represent the first line therapy for generalized anxiety.
For panic disorder, a 2022 network meta analysis of 136 trials by Papola and colleagues in the British Journal of Psychiatry reached a similar conclusion: CBT was the best supported first line treatment.
What this looks like in Bangladesh
In the National Mental Health Survey 2019, published by Alam and colleagues in 2024, anxiety disorders affected 4.7 percent of Bangladeshi adults. Locally, anxiety is often described as “tension” or “chinta”, and many people go first to cardiology or medicine departments because the symptoms feel physical. Academic pressure, job insecurity, marriage expectations and comparison within families are common triggers I see in Dhaka.
What you can do starting today
- Slow your breathing: breathe out longer than you breathe in for a few minutes.
- Notice avoidance. Each time you avoid something out of fear, the fear grows a little. Take small steps toward it instead.
- Limit repeated reassurance seeking and repeated checking of symptoms online.
- Cut down caffeine and late night screen time, which both worsen anxiety and sleep.
- Move your body daily. Even a brisk 20 minute walk helps.
What recovery usually looks like
With CBT, many people notice they are avoiding less and worrying less within the first several sessions. A typical course runs around 8 to 16 sessions, and the skills you learn keep working after therapy ends, which is why the research shows lasting benefit.
Common questions
How do I know if it is anxiety or a real medical problem?
Get medical symptoms checked by a doctor first. That is the responsible order, not something to skip. If tests come back clear and the symptoms keep recurring, particularly in specific situations or without a clear trigger, anxiety becomes a reasonable next explanation to explore.
Can anxiety go away on its own?
Mild, situational anxiety often does ease once the situation passes. Anxiety that has been persistent for months, or that is interfering with work, sleep or relationships, rarely resolves without some form of active treatment.
Is medication necessary, or can therapy alone work?
For many people, therapy alone is enough, particularly for mild to moderate anxiety. Medication becomes more relevant when symptoms are severe, function is significantly impaired, or therapy alone has not produced enough change. That is a decision to make with a psychiatrist, not something to self diagnose toward or away from.
Can anxiety be treated without medication?
Yes. For many people, CBT alone is enough, and research shows its benefits last after treatment ends. Medication becomes more relevant for severe anxiety and is decided with a psychiatrist.
