A client I worked with survived a road accident that killed someone else in the vehicle. Physically, he recovered within weeks. Emotionally, a year later, he still could not sit in the front passenger seat of a car without his heart racing. That gap, full physical recovery alongside ongoing psychological injury, is one of the most misunderstood parts of trauma, and it is part of why PTSD often goes unrecognized here for a long time after the event itself.

What PTSD actually is

PTSD can develop after any event involving real or perceived threat to life, accidents, assault, natural disaster, war, or repeated abuse. Core symptoms include intrusive memories or flashbacks, avoidance of reminders, negative shifts in mood or beliefs about the world, and a persistent state of heightened alertness. It is a normal nervous system response to an abnormal event, not a personal weakness or a sign someone could not handle it.

What the evidence says actually works

A 2023 Cochrane systematic review on psychosocial interventions for survivors of rape and sexual assault concluded that EMDR is a first line treatment for PTSD alongside other trauma focused psychotherapies, including Cognitive Processing Therapy and Prolonged Exposure. A 2025 systematic review and meta analysis in the British Journal of Psychology, examining evidence published since the 2018 NICE guidelines, confirmed EMDR was significantly more effective than waitlist or usual care, with no significant difference in outcomes between EMDR and trauma focused CBT. Both approaches are recommended as first line treatments in major clinical guidelines, including NICE in the United Kingdom and the American Psychological Association. Generic supportive counseling, or simply talking about the event repeatedly without a structured trauma processing approach, has weaker evidence and can occasionally worsen symptoms if it is not paced correctly.

Why this matters specifically in Bangladesh

Road traffic accidents, workplace injuries, and domestic or community violence are all common sources of trauma here, and they are frequently treated purely as physical events. Medical follow up happens, psychological follow up usually does not. There is also a cultural tendency to expect people to move on quickly after surviving something, which can leave genuine PTSD symptoms dismissed as the person dwelling on it rather than recognized as a treatable condition.

What the latest research says

How long does PTSD last without help? The WHO World Mental Health surveys, analysed by Rosellini and colleagues in 2018 using data from 1,575 people with lifetime PTSD, found that about 20 percent had recovered within 3 months, 50 percent within 2 years and 77 percent within 10 years. In other words, half of the people still had symptoms two years after the trauma, which is a long time to suffer when effective treatment exists.

What this looks like in Bangladesh

Road accidents, workplace injuries, violence at home or in the community, and the aftermath of floods and cyclones are common sources of trauma in Bangladesh. Physical injuries are treated, but psychological follow up rarely happens. People are often told to forget it and move on, and when they cannot, they blame themselves. PTSD is not weakness. It is the nervous system stuck in danger mode, and it can be retrained.

  • Avoid using alcohol, sleeping pills without prescription or other substances to numb feelings.

What recovery usually looks like

Trauma focused therapy is usually short and structured, often around 8 to 16 sessions. The first sessions focus on safety and stabilization before any processing of the memory, and many people notice nightmares and startle responses easing as therapy progresses.

Common questions

How long after a traumatic event can PTSD develop?

Usually within the first few months, though delayed onset PTSD, appearing six months or more afterward, is also recognized and real. There is no fixed window after which it is too late to identify or treat it.

Is it normal to have some symptoms after trauma without it being full PTSD?

Yes. Many people experience some symptoms in the weeks following a traumatic event that ease naturally without treatment. PTSD is diagnosed when symptoms persist for more than a month and meaningfully interfere with daily functioning.

Do I have to talk about the traumatic event in detail to get better?

Trauma focused treatment does eventually involve processing the memory in a structured, paced way, but a competent therapist builds toward that gradually and never forces detail before you are ready. It should not feel like being made to relive the worst moment on command.

Will PTSD go away on its own?

Sometimes, but slowly. In the WHO World Mental Health surveys about half of people with PTSD had recovered within two years, so many people suffer for a long time without treatment. Trauma focused therapy usually shortens that path.
