I get messages from Bangladeshis living in London, Dubai and Sydney, sometimes at three in the morning their time, asking the same nervous question. Does online therapy actually work, or is it a compromise for people who cannot get real, in person help. I understand the hesitation. A video call does not feel the same as sitting in a room with someone. But the research on this is more settled than most people assume, and it does not match the doubt.

What the research actually says

A 2025 multilevel meta analysis published in the Journal of Medical Internet Research compared video based psychotherapy with face to face delivery across a range of mental health conditions and found no clear evidence of a difference in symptom outcomes between the two formats. This builds on earlier work, including a large meta analysis of randomized controlled trials on synchronous teletherapy that found no significant difference in outcomes or attrition between video based and in person treatment, and a 2023 JMIR systematic review that reached a similar conclusion for PTSD, mood and anxiety disorders. Studies on therapeutic alliance, the working relationship between client and therapist, have also found ratings in teletherapy to be broadly comparable to in person ratings. The format itself does not appear to be what determines whether therapy works. What matters is the quality of the relationship and whether the approach fits what you are dealing with, the same two things that matter in a physical room.

Where online therapy is not the right call

I want to be honest about the limits, because a lot of online therapy marketing pretends there are none. If you are in a mental health crisis, at risk of harming yourself, or experiencing symptoms of psychosis, online sessions are not the appropriate first response. That needs in person, sometimes emergency, care. Online therapy also gets harder without a stable connection or a private space at home, and that is a real barrier for many clients in Bangladesh, not a minor inconvenience.

Who tends to benefit most

What a session actually looks like

Mostly identical to an in person session, just over video, usually WhatsApp video or a video link, forty five to sixty minutes, with the same confidentiality expectations. The practical differences sit on your side. Find a private space, use headphones if you are not alone, and treat it as a real appointment rather than something you multitask through.

A 2018 systematic review and meta analysis by Carlbring and colleagues in Cognitive Behaviour Therapy directly compared guided internet based CBT with face to face CBT across 20 studies and 1,418 participants, and found equivalent overall effects. The therapeutic relationship also seems to work online: a large meta analysis of 295 studies by Flückiger and colleagues found the link between the client and therapist relationship and outcome was about the same for internet based and face to face therapy.

What the latest research says

Two large meta analyses support online therapy. Carlbring and colleagues compared guided internet CBT directly with face to face CBT in 20 studies with 1,418 participants and found equivalent overall effects. Flückiger and colleagues, analysing 295 studies, found the link between the client and therapist relationship and outcome was about the same for internet based therapy as for face to face therapy. For OCD in young people, a 2024 meta analysis of 71 trials found ERP delivered remotely as effective as in person ERP.

What this looks like in Bangladesh

With specialists concentrated in Dhaka and long travel times within the city, online therapy has become a practical lifeline for people in other districts and for the large Bangladeshi diaspora in the Middle East, Europe and North America. Many prefer a therapist who understands Bangladeshi family life, language and expectations, even when living abroad.

Common questions

Is online therapy as confidential as in person therapy?

Yes, the same principles apply. The one thing worth checking with any provider is what platform they use and whether it is reasonably secure. Ask if you are unsure.

Can I get a prescription through online therapy?

Not from a psychologist. Psychologists do not prescribe medication regardless of format. If medication is needed, that requires a psychiatrist, who can often also consult online, though prescribing across borders has its own legal limits worth checking.

Is online therapy cheaper than in person?

Sometimes slightly, since there is no clinic overhead for that session, but the difference usually is not large. Fees should reflect the psychologist’s time and expertise either way.

How do I know if online therapy is working?

The same way you would judge in person therapy. Are symptoms genuinely easing over weeks, not sessions. Do you feel understood. Are you gaining tools you actually use outside the session. If none of that is happening after a reasonable number of sessions, say so directly to your therapist.

Can Bangladeshis living abroad take online therapy?
